Lost my doctors perscription form?

Hey, after coming out of the Hospital after having a Limited colonoscopy I was given a prescription form for Pentasa 3mg for 28 days.. However i have no idea how but somehow managed to actually lose this on the way home. So the question is if I call into the local GP and show my results and tell them that I have lost my prescription form would I be able to get a replacement form?
