How do people get enough painkillers to sell them?

Ive been watching intervention and i am seriously confused because i cant figure out how the people who sell these large amounts of opiates can consistently have the amount of drugs their clients are looking for. how do they get that many pills on a regular basis? and how do the abusers get into contact with them initially?
